What is Personal Home Health Care?

Personal Home Health Care helps people of all ages remain in the comfort of their own home. From the pediatric patient to the frail elderly adult and all those in between, services are provided which are suitable to the patient’s needs, as prescribed by their physician. Home care allows the patient to recuperate from an illness, receive long term care for the chronic patient, assist the patient with activities of daily living, and so much more.

Our professional nurse will do a comprehensive assessment, consult with your physician, and come up with an individually designed plan of care. The professional services are delivered by our very caring and competent nurses, our certified nursing assistants and home health care aides.

Our aides have been trained to help with activities of daily living like bathing and dressing, and the chores like cooking and light housekeeping. They can monitor vital signs and remind patients to take their medications. In addition, they provide companionship which is so important emotionally to the person in their home.

The plan of care can be limited to a few hours a day or, depending on the patient’s needs, a longer shift. Our nurses are available 24 hours a day to answer your questions and take care of any situations which may arise.

How Do You Pay?

Most home care charges are paid by the government through its Medicaid program. In addition, some services are paid for by private insurance. We also accept private pay.

Carolina Hearts proudly serves the following counties in North Carolina: Scotland, Robeson, Richmond, Anson, Montgomery Moore, Hoke and Lee counties. And we are also licensed to see patients in the following counties in South Carolina: Marlboro, Dillon, Chesterfield, Kershaw, Lee, Darlington and Florence counties.
